Three formats
Every mission refuses differently
Schengen refusals arrive as the standard form in Annex VI of the Visa Code, with numbered boxes ticked and no reasoning beyond them. UK refusals run to six sections and cite the Immigration Rules paragraph behind each ground. A US non-immigrant refusal is usually one slip citing section 214(b) and nothing else.

Confirm, never assume
You confirm what was read before anything is built on it
Parsed grounds are shown for you to check and correct. A plan built on a misread ground is worse than no plan, so the wizard will not move past this step on its own.
The plan
Add, replace, explain, keep
Each action answers a sentence in your notice, and says which one. Things the notice did not question are marked keep, so you do not rebuild a file that was already fine.
- Add: evidence the notice says was missing
- Replace: a document that did not do the job it was there to do
- Explain: something true that reads badly without a sentence of context
- Keep: the parts the caseworker did not question
